The Culture Change Accountability Board is now accepting applications for the 2012/2013 cycle.  Applications are due to Penny Cook by Friday, March 9, 2012.  The Grant Announcement, Application and Deliverables can be found below.  The application AND deliverables must be completed.

Congratulations to the 2011 Cycle 3 Recipients of the Nursing Facility Culture Change Grants!

• Colorado Culture Change Coalition- $30,200 

      

• Eben Ezer Lutheran Care Center- $4,000

• Colorado State Veterans Home at Fitzsimons- $3,270
